In addition, we are thrilled to announce the creation of new support toolkits for headache and constipation — a key outcome of last year’s successful co-management pilots. These toolkits are designed to enhance clinical decision-making and streamline patient care, offering comprehensive resources such as clinical algorithms, visit templates, patient treatment/action plans, and patient education tools. They will soon be accessible on the PHN website, ensuring that these valuable resources are readily available to support your practice.
